Facial paralysis can be a psychological disaster to the affected patients, while facial reanimation still poses a great challenge to the reconstructive microsurgeon. Some controversies related to facial paralysis reconstruction are: (1) One stage vs. two stage reconstruction;, (2) The donor muscle selection: temporalis vs. gracilis vs. pectoralis minor muscle vs. others;, (3) The neurotizer selection: Ipsilateral vs. contralateral facial nerve vs. masseter nerve vs. spinal accessory nerve vs. hypoglossal nerve vs. ipsilateral C7 branch;, (4) The placement of the muscle;, (5) The tension setting of the transferred muscle et cetera.
